recent onset, which was unrelated to exertion. The onset was insidious 
with gradual progression. The patient was a nonsmoker and did not give a
 history of fever, cough, or dyspnea. On examination, equal air entry 
was noted bilaterally with no added sounds. Blood counts and serum 
chemistry levels were within normal limits. A chest radiograph (Figure
