The search for effective treatment of vein graft atherosclerosis has been a paramount challenge for cardiovascular medicine. Experience with new percutaneous interventional devices and pharmaceutical agents has been disappointing. Likewise, application of percutaneous intervention to carotid artery stenosis has been limited by the neurological consequences of cerebral embolization. To address the problem, an exciting new approach is to use a mechanical device to prevent particulate matter from traversing distally. This article reviews the promises of novel distal protection devices to reduce the risk or ameliorate the sequelae of embolization during percutaneous revascularization.
