The finding of subcallosal striations on sagittal thin slice FLAIR images is the earliest imaging manifestation of multiple sclerosis. Gadolinium-enhanced T1-weighted images are currently the most sensitive imaging technique for following multiple sclerosis, particularly for patients on the new immunotherapies.
