Optiray® (Ioversol Injection), a non-ionic, monomeric X-ray contrast medium, has been associated with a high imaging quality, being rated as excellent, good, or adequate in 99.9% of examinations based on an observational cohort study involving over 101,000 cases. This product has been on the market for 30 years, providing clinicians with confidence in its safety and effectiveness.

Optiray® is available in various formulations including prefilled syringes, glass vials, and pharmacy bulk packages, allowing for flexibility in clinical settings. To date, more than 230 million doses of Optiray® have been distributed globally since 1989.

The product offers several physical characteristics depending on the formulation. For instance, Optiray 320 contains 320 mg/mL of iodine with an osmolality of 702 mOsm/kg, while Optiray 350 has 350 mg/mL of iodine and an osmolality of 792 mOsm/kg. The viscosity at 25˚C for Optiray 320 is 9.9 cps, and for Optiray 350, it is 14.3 cps.

Several dosage forms are available for both adult and pediatric patients. The indications for adult patients include cerebral arteriography, peripheral arteriography, and CT imaging of the head and body. For pediatric patients, indications include angiocardiography and CT imaging of the head and body. It’s important to note that Optiray is contraindicated in patients with symptomatic hyperthyroidism.

Warnings and precautions include the risks associated with inadvertent intrathecal administration, which can lead to serious adverse outcomes such as convulsions and acute renal failure. The product can also cause significant hypersensitivity reactions, including anaphylaxis, especially in patients with a prior history of contrast reactions.

In terms of adverse reactions, nausea is the most common reaction observed in adults, while pediatric patients may experience similar reactions. Postmarketing experiences have documented various other adverse events, including anaphylactic reactions, cardiovascular complications, and severe cutaneous adverse reactions.

Caution should be exercised regarding its use in pregnancy and lactation, as the data on potential risks are limited, and iodinated contrast agents are known to be excreted in minimal quantities in human milk.
